I will never believe it was premeditated, but there had to be a certain feeling or lack of towards them, to allow him to snap so thoroughly as to destroy them, and not just his narcissist abuser wife; I believe there's a higher rate of abuse of premies that spent a long time in hospital, from lack of bonding, fretfulness, whatever could make a sleep deprived person turn on a little child.
