Cosmetics Not Unlocking

I've been trying to unlock the hooded helmet for Bulwark: "Achieve victory in 5 Missions in Operations mode or matches in Eternal War mode to unlock." I've done 3 Operations so far and I've amassed well over 5 wins in Annihilation matches, but the helmet still hasn't unlocked.

I've also noticed I haven't unlocked other items I should have, like the winged sword Storm Shield for 4 Operations or Eternal War matches, the haloed skull(1 victory), laureled skull(1 victory) and shrouded laurel skull(8 victories) pauldrons, the robed cross chest(7 victories), the skull and chain gauntlets(2 victories) and finally the spiked knee guards(3 victories)

So what am I getting wrong? Do I need to be playing certain EW modes or on certain Operation difficulties? Is it just broken?
