need help with nclex rationales—how do u write them?

hey guys!

i’m studying for the nclex and honestly struggling with rationales. like, i know they’re super important but idk how to break them down or review them properly.

can someone pls share how you do it? like:

• how do u write your rationales?
• what do you include?
• any tips for organizing and remembering?

and if you could drop a sample of how you write yours, that’d be amazing! seeing a real example would help a lot.
